Facial volume loss is one of the earliest and most significant contributors to an aged appearance — and one of the most undertreated. Fillers address it temporarily. Fat transfer can provide longer-lasting volume support in appropriately selected patients, using the patient's own tissue harvested from the abdomen or thighs and transferred in precise micro-aliquots to depleted zones.
At EyeFACE, fat transfer is integrated into a broader anatomical approach — not used as a standalone filler substitute. It is considered within RERF® Surgical Series planning, central to RERF-Sculpt™, and used selectively with OFA-Bleph™ and revision planning when anatomy and safety support it.
Lower eyelid hollowing can come from true volume loss, orbital fat prolapse above a hollow, cheek descent, or a combination. EyeFACE planning separates those causes before recommending fat transfer, fat transposition, or structural lifting.
Facial fat transfer can soften depleted cheek and midface areas, but it works best when it is matched to the tissue position underneath. When descent is the main issue, lifting may matter more than adding volume.
Nanofat and microfat are discussed in the context of surgical planning, tissue quality, and recovery goals. At EyeFACE, fat handling is considered as part of RERF-Sculpt™ and procedure-day coordination when it is clinically appropriate.
How We Decide
Patients often ask for “fat transfer under the eyes” when the visible concern is actually a lower eyelid bag, a tear trough shadow, cheek descent, skin texture, or dark pigmentation. The same shadow can have several causes.
This is why EyeFACE uses facial analysis first: preserve useful tissue, avoid overfilling, and decide whether the best answer is fat transposition, microfat, nanofat, RERF® surgery, RERF-Energy™, or no procedure at all.
